Medical Marijuana & Qualifying Conditions

As defined by Chapter 69.51A of the Revised Codes of Washington (RCW), patients with terminal and/or debilitating medical conditions are allowed to use medical marijuana. These conditions include:
  1. Cancer.
  2. Human Immunodeficiency Virus (HIV).
  3. Multiple Sclerosis.
  4. Epilepsy, seizure disorders, and spasticity disorders.
  5. Intractable pain, unrelieved by standard medical treatments and medications.
  6. Glaucoma.
  7. Crohn's disease.
  8. Hepatitis C, with debilitating nausea and/or intractable pain.
  9. Diseases, including anorexia, which result in nausea, vomiting, wasting, appetite loss, and cramping.
  10. Irritable Bowel Syndrome, Cachexia, and Frybromyalgia.
  11. Arthritis and Degenerative Disk Disease.
  12. Diabetes with complications.

If you have one of the conditions listed above, simply go to the ROI page, download the Release of Information form, fill in the blanks, sign it (NOTE, must have signature or cannot process), and either fax it, or mail it to the address noted on the form. In the event you already have medical records in your possession, simply fax them to us. Please make sure to include a cover sheet with your contact information.

Once we receive your signed form, we immediately fax it to your health care provider. It generally takes no longer than two business days to receive the records. Our doctors then review the records, qualify you, and we schedule you for a clinic, held every Thursday and Friday. Generally, once we receive your fax, it takes about a week to schedule an appointment.
